MONTH TO MONTH:

30%+ $25 PRE AUTH FEE

The contract rate for our month to month services is 30% per claim payout, plus the $25 pre-authorization fee.

RETRO SERVICE:

$100/10 PATIENTS

We extend retro services to our established contracted clients at a rate of $100 per 10 patients. This service is provided for past procedures up to 6 months since date of service.

ANNUAL:

The contract rate for our annual services is 25% per claim payout, plus $25 pre authorization fee.

This is a one time fee charged per patient to verify medical benefits.
